By confirming your appointment, you:

hereby give your informed consent or provide consent to voluntarily and independently participate in an “in-person” or “distant” Reiki treatment offered by lala Cura.

You understand clearly and completely that the services provided at and by lala Cura during these sessions are in no way intended to be a substitute or replacement for past, current, or future diagnosis, therapy, treatment (including medication and diagnostics), intervention, protocol, interpretation, recommendation, and/or follow-up by/with medical, veterinary, and other healthcare professionals.

You are fully aware that lala Cura will not offer any diagnosis or recommend any medical treatment, diagnostics recommendation or interpretation, or prescribed medication.

Furthermore, you understand clearly and completely that you and/or the animals in your care must continue to have regular medical/veterinary visits, appointments, and/or check-ups as part of an overall personal health care plan; and you should contact your own certified and licensed medical physician/doctor/veterinarian/healthcare professional for any physical or psychological ailments or concerns that you and/or the animals in your care may have in order to get proper medical advice.

You understand clearly and agree completely that your and/or your animals’ participation in these sessions is voluntary and that at any time during the sessions you can choose to end your participation.

You understand clearly and completely that any information exchanged during any session is educational in nature and is to be used at your own discretion. You also understand that any information exchanged during these sessions is strictly confidential and will not be shared with anyone without your written permission.

Finally, you understand clearly and completely that by providing this informed consent you are assuming full responsibility for participating in these sessions and you hold fully and unconditionally harmless lala Cura and the facility/location where the services are provided.
